Apply for The FMA and CNA Safety Awards
Award levels are determined by comparing your company’s injury and illness rate to the national average for your industry (as defined by your NAICS code, from Bureau of Labor Statistics — BLS). Your company qualifies if the rate is lower (i.e., better) than the BLS rate for the same industry. All data submitted will remain strictly confidential and will only be used for evaluation during the judging process.
If your company operates more than one facility, each facility may earn its own award. For that reason, please submit a separate OSHA 300A form for each facility.
Application deadline: March 20, 2026, by 5:00 p.m. CDT.
Winners notified by: mid-April.
